Community Living Stabilization
The St.Amant Community Living Stabilization Service is a comprehensive treatment plan that aims to foster behavioural stability, prevent permanent out-of-home placements and prevent the breakdown of other supports.
Individuals who receive services from the Family Services-Supported Living Program and also experience immediate or imminent behavioural or psychological problems that jeopardize their ability to remain in their homes are eligible for the CLSS.
Please note: Individuals who represent an unmanageable safety risk to other clients or neighbouring communities are ineligible, as the service provides constant supervision and support but is not a locked, secure facility.
Participating individuals may be challenged by a broad spectrum of issues including:
- Aggressive or destructive behaviours
- Temporary life adjustment problems
- Depression
- Other mental health diagnoses
Once a participant is accepted into the program, the CLSS team completes a thorough clinical assessment, develops a comprehensive treatment plan and orients care providers to the details of the plan.
The CLSS team consists of Psychologists, Psychiatrists, Registered Nurse Consultants, Clinical Pharmacists, Behaviour Analysts and Treatment Staff.
CLSS offers services in four streams through a client’s Community Service Worker.
In-House Services
Brief In-House Admission (Max 14 days): Individuals are in a behavioural crisis and will leave their normal home to reside in our specialized unit for a few days.
In-House Stabilization (Max 90 days): Individuals are experiencing a behavioural crisis and their formal support network will not be able to re-establish supports without external assistance. Key elements include residential care, comprehensive assessments, development of treatment and safety plans, orientation to members of the support network and follow-up upon return to the community.
Outreach Services
Comprehensive Clinical Outreach Assessment: Individuals are not experiencing a crisis but are near one. In these situations, the individual remains in his or her home, where the clinical team completes a thorough assessment of factors contributing to the client’s behavioural presentation and provides recommendations to restore and maintain stability.
Clinical Outreach Treatment Consultation: Once a client has been served through In-House Services or Outreach Services, the clinical team works with the client’s support network to implement treatment plans in the client’s community-based home.
